Baptism Form
Ray of Hope is eager to be a part of your baptism and walk with as you continue to grow in Christ. Our baptisms are conducted on the 4th Sunday of each month with the exception of July and December, due to the schedule of events at the church.
Please take the time to review and complete the form below in its entirety. This form must be submitted at least ONE MONTH prior to your requested date of baptism. One form should be submitted per candidate. Once your information is received, an administrative staff member will contact you to confirm the details of your baptism. If you have any questions, please feel free to contact the church office at (770) 696-5100. Our office hours are Monday-Wednesday from 9:00am-6:00pm.
